Cholesterol Tracker
Instructions:
Print out this Cholesterol Tracker chart.
Take it to your next doctor appointment, and ask the doctor for your cholesterol numbers. Write them into the My Baseline column.
Take the tracker to each appointment with your doctor. When your doctor checks your cholesterol, write in your new numbers. You want to see your HDL numbers rising and your LDL, triglycerides and total cholesterol numbers falling over time. If they are, keep up the good work! If they're not, talk with your doctor about other steps you can take.
Use the Notes section to jot down
- New medications or new dosages
- New exercise, diet or other lifestyle changes you made
- Side effects
- Other observations
